Record the date, provider, reason for visit, instruction or restriction, follow-up, and source filename. Consistent fields make missing periods easier to spot.

Include non-visit events

A referral, cancelled appointment, pharmacy issue, record request, or insurer authorization can explain a gap. Label the event accurately.

Link costs carefully

Connect each bill and insurance explanation to the service date when possible. Keep billed, paid, adjusted, and outstanding amounts in separate fields.

Correct your index when new information arrives but preserve source records. If a medical record appears wrong, use the provider’s correction process rather than changing the downloaded record.
